Health Form
When your child takes part in a camp or activity outside normal meetings, we need a new health form for that event. It is essential that we have accurate and up-to-date health and contact information for each member.
Please note that this is not our rule, it is a rule set by the Scout Association.
For each activity, please download and print the health form and bring it with you. We do try to bring spare forms with us in case anyone forgets, but parents often don't remember doctor's address details and NHS numbers, so it's far better to fill in the form at home and bring it with you.
Please include as much information as possible, even if you don't think it is relevant or important - it may be more important than you think!
If you do not know your child's NHS number, please contact your doctor's surgery. They will definitely know.
Tip: Save a copy of the health form on your computer, type in all the details and save it. Next time you need a copy of the health form, it will be ready to print, sign and date!
